About Us:
Agape of Appleton is a non-profit human services agency providing residential support services to individuals with differing abilities. We are looking for a compassionate and dedicated Residential Case Manager to join our Agape 7 CBRF program. This is an exciting opportunity to provide case management, program management, staff supervision, and direct care support to individuals in need.

Position Summary:
As a Residential Case Manager, you will lead the Agape 7 CBRF program by ensuring the proper care and support of individuals. Your role includes developing Individual Support Plans (ISPs), managing staff, providing advocacy, and supporting residents with their daily needs and challenges. You will be an integral part of a supportive and diverse team that values kindness and community integration.

Key Responsibilities:

  • Provide inter-agency and intra-agency case management for individuals.
  • Develop, update, and implement Individual Support Plans (ISPs) and ensure appropriate services are provided per contractual agreements.
  • Maximize the social functioning and community integration of each individual.
  • Advocate for and safeguard the human and civil rights of individuals while fostering their dignity and self-worth.
  • Guide direct care staff and offer support during challenging behavioral situations.
  • Respond to crises and provide immediate support as needed.
  • Lead and train direct care staff through coaching, mentoring, and performance management.
  • Provide direct care support to individuals based on program needs and staffing levels.

Qualifications:

  • Associate degree from an accredited college in a healthcare-related field or Bachelor's Degree in a related field with experience in healthcare.
  • At least 2 years of previous management experience (preferred).
  • Experience working with individuals in a group home setting is a plus.
  • Must be at least 21 years of age.
  • Maintain a valid driver’s license with a good driving record.
  • Ability to assist participants with their physical and emotional needs.
  • Strong judgment and ability to follow written and oral directions.
  • Flexible and adaptable to changing hours, environments, and situations.
  • Ability to encourage personal growth and development in others.
  • Sufficient experience or education to meet the position requirements or the ability to acquire and maintain such requirements through training.

Program Information:
This position is based in a fast-paced, handicap-accessible, single-story, 8-bed coed home. Some of our participants are non-verbal and communicate in various ways. The ideal candidate should have experience with durable medical equipment and supporting participants' physical needs. Our participants are fun, kind, and a joy to work with!

  • Hourly wage of $21.00 to $25.00 per hour, plus overtime for hours worked over 40 per week.

Additional Information:
