Sequoia Integrative Medical Services is a Wisconsin-based healthcare organization dedicated to bringing high-quality primary care directly to seniors in assisted living communities across the state. By delivering personalized medical care onsite, we help residents receive the attention, support, and continuity of care they deserve in the comfort of their community.
Our multidisciplinary team of physicians, nurse practitioners, and nurses provides comprehensive primary care, chronic disease management, care coordination, and responsive clinical support 24 hours a day, seven days a week. Working in close partnership with residents, families, and facility staff, we create individualized care plans that support better health outcomes, enhance quality of life, and provide peace of mind for those we serve.
