Medication Error Reduction: From Risk to Reliable Practice
Medication errors can quickly become more than a documentation issue. They can affect resident safety, regulatory compliance, staff confidence, and organizational risk. In many cases, errors are connected to knowledge gaps, process variation, documentation challenges, or unclear medication management practices that can be difficult to recognize until a pattern develops.
This practical 1-hour webinar, led by Bethany Everest with Everest Elevation, moves beyond the individual error to examine why medication errors occur and how organizations can build safer, more reliable medication practices.
Participants will learn how to:
• Identify and assess medication errors to determine where risks and breakdowns are occurring
• Use root cause analysis to uncover contributing factors, including knowledge and competency gaps
• Understand key medication-management responsibilities and regulatory considerations
• Strengthen practices surrounding medication receipt, storage, assistance, preparation, documentation, and resident self administration
• Develop targeted staff education and competency validation based on identified needs
• Track and trend medication errors using meaningful measures and KPIs to identify recurring patterns
• Develop and implement corrective action plans and monitor whether interventions are working
• Apply lessons learned from a real-world medication-error reduction initiative and strategies used to sustain improvement over time
Attendees will leave with practical strategies and tools they can apply within their facilities to strengthen medication safety, support staff, reduce organizational risk, and better protect the residents they serve.
