This is a small, personalized, Community Based Residential Facility licensed by the State of Wisconsin Department of Heatlth and Human Services. Private or shared rooms are available for up to five residents. A team approach which utilizes R.N. monitoring and weekly wellness checks is used to create a personalized care plan which meets your loved ones medical, physical, dietary, social, and emotional needs while encouraging and maintaining independence. Friendly CNAs provide around the clock care and supervision.
